Nursing - Clinical, Simulation, and Lab Student Handbook
Nursing Department Clinical, Simulation, & Lab Student Handbook The following content is applicable to all student experiences in clinical, simulation, or lab settings. This handbook is a resource for students in addition to the Nursing Student Handbook, Catalog, course syllabus, and University policies and procedures. Attendance: 1. In the event a student misses 15% or greater of a combination of simulation and clinical hours the student will receive a failing grade for the course. 2. Clinical Student Compliance Requirements a. Student must be clinically compliant by 1200 business day before the assigned clinical date to attend clinical; if the student is not clinically compliant by 1200 the student will not be allowed to attend clinical. i. These missed hours will not be excused, and the student will be at risk of failing clinical due to hours missed. b. If a student is not clinically compliant two times, a Student Conduct Report will be completed. c. If a student attends clinical or is present at a clinical site and is not compliant with ClinicalStudent, a Student Conduct Report will be completed. 3. Faculty must be notified 1 hour before the clinical shift or scheduled simulation if a student is going to be absent. a. Failing to notify Clinical Lead & Clinical Instructor/Simulation Instructor of an absence prior to clinical is unprofessional and will result in 0 points for clinical performance. b. Extraneous circumstances will be handled on a case-by-case basis c. Any student who fails to notify faculty more than 1 time in one semester to any clinical/simulation/lab activity will receive a Student Conduct Report and be reported to the BSN Program Director 4. Students may not attend clinical or simulation if they are ill and/or running a fever >100F. a. Students who are ill will be sent home at the faculty’s discretion for patient/peer protection. b. COVID Policy— i. Refer to the most current Rockhurst University COVID policy. ii. Communication 1. Must be in contact with professors regarding illness severity; this includes theory faculty, Clinical Lead, clinical instructor, simulation, and lab coordinator. c. Although your absences may be excused absences; more than two missed clinical, simulation or lab, will be evaluated by the course leads. 1. May require added remediation or an incomplete. 5. Absences must be approved by faculty and should only be for illness (must have doctor’s note), emergencies (must provide documentation of accident/emergency), death in the family (immediate family-date of funeral), etc. a. Unavoidable Absence i. In case of a student’s inability to attend clinical, simulation, or lab due to personal illness or other unexpected events, students are required to notify their course faculty and clinical instructor/preceptor in writing (email or text) prior to the absence. ii. Students will not be guaranteed the opportunity to make-up missed clinical/simulation/ or lab days unless the absence is related to pregnancy or child. 1. The student may be at risk of failing clinical due to hours missed. iii. At the time of notification, the student may be required to provide documentation to justify the absence. 1. Documentation must be dated for the missed clinical or simulation date. 2. Documentation must be submitted within 48 hours of missed clinical or simulation date. iv. If the absence is necessary or justified, at the discretion of course faculty, an opportunity may be given to help the student meet course objectives. 6. A make-up clinical or assignment may be provided for the student who missed a clinical day due to an approved absence. a. The make-up clinical day will be provided to students at the discretion of the unit and faculty availability. i. Students will be expected to make up excused absences from clinical. Students are expected to attend clinical on days when an alternate clinical day is available. If a student declines an alternative clinical day, the hours will count against their 15% allowable missed combined clinical/simulation time, which could result in course failure. b. Alternative assignments will not be given for missed clinical or simulation hours due to: i. Clinical Student non-compliance ii. The student is unable to provide documentation, within a reasonable amount of time, for approved absences, leaving early/arriving late iii. Unapproved absence c. Missed clinical hours will contribute to the 15% allowable missed clinical/simulation/lab hours for this course. d. If an alternate clinical assignment is completed for more than 2 clinical/simulation absence occurrences or >15% of the clinical/simulation/lab hours, continued course progression will be evaluated by the lead clinical faculty and BSN Program Director. 7. Late/Tardy a. Late is defined as arriving after the scheduled meeting time. b. Any student who is late more than 1 time in one semester to any clinical/simulation/lab activity will receive a Student Conduct Report and be reported to the BSN Program Director c. Students may not arrive at the clinical after 0730. d. Students will not be permitted to participate in simulation if greater than 30 minutes late. 8. No Call, No Show a. No call, no shows are unacceptable in professional nursing practice b. In severe emergent situations, the student should call course faculty and clinical instructor/preceptor as soon as possible to discuss the reason for the absence c. If a student does not call, or show for clinical, simulation, or lab; a Student Conduct Report will be completed. i. Student may be at risk for failing clinical/simulation due to missed hours and/or missed points. 9. Clinical Readiness a. Students may not participate in clinical if non-compliant with clinical readiness requirements as listed in Clinical Student. b. Contact the Clinical Readiness Coordinator with questions. c. Students are required to meet the requirements of the assigned clinical site; some clinical sites may have additional requirements. d. Students are required to meet all MOKAN requirements. e. If the student is not active in an assigned clinical setting for a semester (Fall or Spring), the student is required to complete a “Clinical Skills Refresher” before the student’s first assigned clinical date. i. Students are required to follow the Clinical Skills Refresher policy. ii. If the Clinical Skills Refresher is not completed before the student’s first assigned clinical date; the student will not be allowed to attend clinical-- These missed hours will not be excused, and the student will be at risk for failing clinical due to hours missed. Professionalism: 1. Students are to act in accordance with the policies and procedures in Civil and Professional Behavior as outlined in the Student Handbook. 2. Students must adhere to the Academic Integrity Policy. 3. Students in violation of Rockhurst University policies are subject to loss of points, failure of assignment and/or failure of course (to be determined by course lead and BSN Track Director). Professional Appearance: 1. Students are expected to dress in accordance with the college’s Student Professional Appearance Policy (provided in the Learning Management System). 2. Students are also expected to follow the MOKAN guidelines policy and/or any special instructions from clinical sites. 3. Points will be deducted for deviation from the policy at the instructor’s discretion. 4. Students are also at risk of being sent home if appearance is unacceptable a. Student will not receive any points for that clinical day and will be at risk of failing clinical due to missed hours. b. Students will receive one warning, after the second warning, the student will be sent home; this will be an unexcused absence. Cell Phones & Smart Watches: 1. Students MAY NOT use their cell phone in the clinical or simulation area. Clinical site policies do not allow cell phones in patient care areas. Students should provide family members, daycare, etc. with the facility phone number if they need to be reached in an emergency. Students are not allowed to carry cell phones. Keep cell phone in a locked area. In addition, smart watches are not allowed in clinical areas. 2. Students MAY NOT wear smart watches in clinical or simulation areas. 3. Students using their cell phone or smart watch in the clinical areas will receive a warning for the first offense. After receiving two offenses, a Student Conduct Report will be completed. 4. Student MAY NOT bring or use laptops, iPads, tablets, etc. at the clinical sites/clinical and simulation areas. 5. Student will also lose Professionalism points for that day. Leaving Clinical: 1. Students may not leave the clinical site during clinical contact hours for any reason (bring your lunch or plan to purchase lunch from the cafeteria if available). 2. Leaving the clinical site without communicating with Clinical Instructor will result in a Student Conduct Report; and will not receive all points for that clinical day and will be at risk for failing clinical due to missed hours. Safety 1. Refer to MOKAN Clinical Orientation Manual a. All clinical students will be required to follow policies as outlined in the Clinical Orientation Manual developed by CNE (Collegiate Nurse Educators of Greater Kansas City) and KCANE (Kansas City Area Nurse Executives) 2. Refer to Clinical and Simulation Remediation policy. a. Two remediations are grounds for dismissal or failure of clinical. Weather Policy 1. Students will be notified via e-mail by Clinical Lead of any changes to the clinical schedule. 2. In the event that RU closes campus or cancels classes, you may still be required to attend clinical. 3. Cancellation will be decided on a case-by-case basis. a. Students will be approved to attend clinical if: i. Clinical site is open. ii. Clinical site is willing to take student. iii. Clinical instructor able to attend. 4. In the event that SLCNHS decides to cancel school after the students have arrived at their clinical location and they have not closed; the assigned faculty and the clinical lead will look at each location individually and will decide if the clinical group will stay or leave. a. Any hours missed will then be deducted. 5. Students are expected to attend clinical unless notified. Late Work 1. 10% deduction to overall assignment grade if submitted within 24 hours after assigned due date 2. 20% deduction to overall assignment grade if submitted 24 hours after assigned due date 3. 0% will be received for assignment is submitted after 48 hours 4. Correct documentation/forms must be submitted in order to receive credit. a. If the correct document or assignment is not uploaded and is past the 48 hours; a zero will be earned. 5. ALL post-clinical and post-simulation documentation must be submitted in order to pass the course. HIPAA 1. Students must maintain compliance with the Health Insurance Portability and Accountability Act (HIPAA). 2. See HIPAA compliance section of the syllabus, Nursing Student Handbook, and refer to PowerPoint in the Learning Management System for more information. 3. Clinical Assignments a. Please refer to the PHI Protection Policy and Procedure in the Learning Management System Parking 1. Students will park in designated areas (to be provided by Clinical Lead). 2. Students found to be NOT parking in designated areas will be asked to move to correct areas a. This may lead to the student being tardy to clinical b. Will lose professionalism points c. Students must attend at least 85% of clinical/simulation contact hours AND achieve at least 75% on graded evaluation activities as described in the learning assessment methods above. Medication Calculation Exam 1. Students must pass the medication calculation exam with a > 90% to pass the clinical course (3 attempts allowed). 2. This will be administered during your theory course time. a. Students must pass the Medication Calculation Exam in order to pass medications at clinical. 3. Students must pass the Clinical-Medication Review quiz with a >90% to pass medication at clinical. a. Student will not be allowed to administer any type of medication until a >90% has been achieved. b. Clinical-Medication Review quiz will be given at the beginning of each clinical orientation. c. If the student does not achieve a >90% on the Clinical-Medication Review quiz after two attempts, the student will be assigned remediation. i. If the student does not achieve a >90% on the Clinical-Medication Review quiz (3rd attempt) and after remediation, the student will be assigned remediation for a second time. ii. If the student does not achieve a >90% on the Clinical-Medication Review quiz after four attempts and two remediations, the student will fail clinical. Clinical Unit Assignments & Simulation Groups 1. Clinical and simulation groups will remain as assigned throughout the semester. 2. Students may not request to switch clinical or simulation groups. 3. Please refer to group assignments as posted in Canvas 4. Placements are final unless students are otherwise notified. All placements are subject to change based on availability and class sizes. Students will be notified of any changes. 5. If a student is asked to leave his/her assigned clinical site/unit and/or not return to assigned clinical site/unit, the student will not be given an alternative site. a. The student may receive additional consequences as outlined in the student handbook and may be at risk for failing clinical due to missed clinical hours. Lab Specific Attendance: 1. Attending skills lab for at least 2 hours per week is required to pass skills lab. a. Activities that contribute to the required 2 hours/week in skills lab include participation in skills demo, practicing skills, and skill checkoffs. i. Outside work, including other courses or clinical prep work should not be done during lab hours. ii. ATI modules can be completed during the required 2 hours/week if completed in the skills lab. b. Missed lab hours will contribute to the allowable 15% missed clinical/simulation/lab hours for this course. c. Participation points i. 10 points are awarded for each 2 hours/week successfully completed. ii. Hours may not be compiled weekly unless approved by lab faculty. iii. If less than 2 hours are completed in the lab per week, scoring is as follows: 1. 0-59 minutes: 0 points 2. 60-119 minutes: 5 points 3. 2 or more hours: 10 points iv. Participation points are available for J1 clinical immersion in weeks 1 and 2. 2. Faculty must be notified within 1 hour of scheduled time if student will be absent or late. a. Students over 5 minutes late to a skill check-off or skill demonstration are responsible for changing the day or time for skill activity within the lab schedule. b. Failure to notify faculty or remove student name from the lab schedule within one hour of scheduled activity time may result in a 10-point deduction from associated skill check-off. c. Extraneous circumstances will be handled on a case-by-case basis. 3. Makeup work a. If a student is absent for an approved absence longer than 3 days, an extension may be granted up to one week for skills lab requirements including skill demonstrations, required hours, and skill checkoffs. b. Makeup work will be granted on a case-by-case basis and does not apply to unapproved absences. 4. Unapproved Absences a. The student is unable to provide documentation within 48 hours regarding an approved absence b. Student is arriving late more than once c. 2 or more unapproved absences will result in a Student Conduct Report and be reported to the BSN Program Director. Clinical Readiness: 1. Skill Checkoffs a. Each skill checkoff is worth 25 points. i. A score of 0 points will be given to the student if skill competence is not displayed before the due date or after 2 failed attempts. b. Each student will have access to the evaluation standards required for each skill checkoff and are encouraged to use these standards in practicing the skill. c. Each student must have a peer check-off completed and signed prior to attempting an individual skill check-off with faculty. i. Students will not be permitted to check-off on any skill without a completed peer check-off first. d. Each student will receive 2 attempts at passing a skill check-off. i. If a student must repeat after the first attempt, they are allowed a second attempt with no point deduction. ii. If a student needs to repeat a skill checkoff, the student must wait at least 24 hours from the initial attempt to attempt the skill checkoff again. iii. Failure to complete a skill check-off within the allotted 2 attempts will result in a score of 0 for the skill. 1. Even if a 0 is scored for any skill, the student must display skill competence before the end of semester to pass the lab portion of this course. e. Skill checkoffs cannot be completed the same day as skill demonstration. It is imperative that students take time to master each skill following the demonstration. f. Following completion of the skill checkoff, both the student and lab faculty will sign the skill checkoff sheet indicating whether the student passed, requires repeating, or failed the skill. i. If a skill requires repeating, the student is responsible for bringing the initial checkoff sheet(s) to any repeating skill checkoffs for that skill. g. Completed skill checkoffs sheets are to be uploaded to Canvas before the designated due dates/times. h. Students may wear appropriate casual wear or street clothes for skill demonstrations and practice time in the skills lab. i. Students must be in full clinical attire for skill checkoffs with lab faculty. This includes school issued scrubs, hair pulled back, appropriate footwear, minimal/natural makeup, ID badge, etc. Scrubs should be free of wrinkles and hair. Professionalism: 1. Attitude/Communication a. A professional attitude and communication between faculty and students and between students is required in the lab. b. Unprofessional actions including yelling, vulgar language, poor attitude, or any form of violence are not permitted in the lab and will result in dismissal from the building and direct reporting to the BSN Program Director. Lab Safety: 1. Children are not allowed in the lab at any time due to safety concerns. 2. Food and drink are not allowed in the lab. 3. Use of needles occurs in the lab and must be done under the supervision of faculty. Equipment Guidelines: 1. Students must bring purchased lab bags/supplies to lab for practice and skill checkoffs. 2. If any backstock of supplies is used, the student is responsible for replacing the equipment where it was found for further use. 3. Manikins are provided to assist in the learning process of psychomotor skills and should be treated gently. a. Only water-soluble lubricant (provided in lab) can be used on the manikins. b. If lubrication is used, the student is responsible for cleaning the manikin using water and dish soap if needed. c. Manikins in the skills lab should not be injected into. Additional materials are available to practice injections. Lab Schedule: 1. The lab schedule will provide demonstration and skill check-off time slots for J1 and J2 students to sign up on within a shared google document. a. Students are each responsible for signing up for a demonstration and skill check-off time slot promptly. i. Each skill checkoff appointment is 10 minutes in length. ii. Each demonstration block is 1 hour in length. iii. If the student cannot attend the selected time slot, they are responsible for removing their name from the sign-up sheet. iv. Students that do not remove their name from the sign-up sheet, do not notify faulty, or do not show, will receive a 10-point deduction from the associated skill check-off. b. If students are unable to find a time slot that fits their schedule, they may reach out to faculty for assistance. c. Demonstrations are conducted by faculty and are limited to 10 students per time slot. i. Walk-ins are welcome but students will still be capped at the 10-student limit unless approved by lab faculty. ii. Students are expected to sign in for each demonstration they attend. d. Students are responsible for allotting enough time to practice each skill.
